An empty or unfinished grave counts -3. A finished grave counts its headstone plus its fence, minus the red skulls of the body inside, and never more than that body's white skulls: a Marble Cross II and Marble Fence II (+14) on a corpse with 3 white skulls still count 3. Match the decoration to the body.
Headstones
Crafted at a workbench, then placed on a grave. The number is what the piece adds to its grave, up to the cap set by the body's skulls.

Graveyard questions
Which decoration gives the most quality for the least work?
Per object, Columbarium II (+20) is the largest single step. The fence and road upgrades add a flat amount to the whole graveyard once each (+10, +20 and +10).
Why did a better headstone not raise the grave?
The grave was already at its cap. A grave's value is its headstone plus fence minus the body's red skulls, and it can never exceed the body's white skulls. Past that point a better headstone adds nothing; save it for a body with more white skulls.
Do the graveyard areas add quality?
No. Grave, walkway, columbarium and mixed areas cost nothing and add a 12 x 12 space to build in; what you place inside is what counts.
Where are graves themselves made?
An Empty Grave is placed from the graveyard blueprint desk at no cost. The headstone and the fence are crafted items: wooden ones at the Woodworking Workbench, stone and marble ones at the Stonemason's Workbench.
